Revolutionizing Therapy 

Getting Started:

All therapy services require a current doctor’s order (or script) for evaluation and/or treatment.

If you are interested in becoming a patient at Progressive Step, please contact our office during business hours or discuss your concerns with your doctor who can prescribe an evaluation and/or treatment.

Appointments:

Therapy appointments are set according to patient and therapist availability and prior commitments / schedules. Most therapy appointments are scheduled 1-3 times weekly for 30-60 minute sessions based on therapist recommendation and patient availability. Appointments are booked as standing appointments and are usually carried over from week to week.

We make every possible effort to schedule patients at times that are most convenient and do take late appointments as needed.

Intake:

New patients are required to complete a set of intake forms, provided to the right.  Please, print the set of intake forms and fill them out as completely as possible. Completing the forms before your visit will speed the intake process.

For your convenience, if you are scheduled for an evaluation, a case history form is also provided. 

On your first visit, you should bring:

• All completed forms

• Your insurance card

• Driver’s license (or other photo ID)
